Duties And Responsibilities
- Start of Care and Visitations: Initiate patient services, develop personalized care plans, and conduct home health treatments as needed. Perform initial assessments, coordinate with healthcare providers, and ensure high-quality care through regular home visits.
- Clinical Management: Oversee the maintenance of patient clinical records, statistics, reports, and records for purposes of evaluation and reporting of organizational activities.
- OASIS Assessment & Documentation: Conduct and oversee accurate and timely OASIS assessments, ensuring they meet CMS requirements.
- Staff Supervision: Assure proper maintenance of clinical records in compliance with local, state, and federal laws. Provide ongoing education and resources for staff on proper OASIS data collection, updates, and reporting.
- Regulatory Compliance: Ensure compliance with all regulatory agencies governing health care delivery and the rules of accrediting bodies by continually monitoring operations and reporting and initiating changes where required.
- Quality Improvement: Participate in the development and implementation of the strategic plan for the home health service line.
- Planning & Evaluation: Plan, organize, direct, and evaluate operations. Participate in the review, analysis, and appraisal of the effectiveness of the total Agency program.
- Compliance: Act as corporate compliance officer and HIPAA Privacy Officer.
- Fiscal Planning: Undertake fiscal planning by budgeting and managing operations in accordance with established fiscal parameters.
- Disaster Coordination: Serve as the agency’s Disaster Coordinator.
- On-Call Duty: Be available on-call to handle any emergencies or issues that arise outside of normal working hours. This includes responding to urgent patient needs, coordinating with healthcare providers, and making critical decisions to ensure patient safety and continuity of care.

- Education: Must be a Registered Nurse (RN) in the state of California.
- Experience: At least 3 years of experience in health care management and home health care operations is required.
- Certifications: Current CPR certification.
- Skills: Excellent organizational, interpersonal, and communication skills.
- Proficient Skills: Proficiency in the use of Electronic Medical Records (EMR).
- Leadership: Strong leadership and supervisory skills, with the ability to motivate and inspire a team.
- Decision Making: Excellent clinical judgment and decision-making skills. In-depth knowledge of home health care regulations and best practices.
- Strong communication and interpersonal skills.
- Proficiency in relevant healthcare software and documentation systems.
- Competitive salary range: $103,400-$160,140 (based on experience and qualifications)
