What does a Hospice Social Worker do? Here are some key responsibilities:
- Complete the comprehensive psychosocial assessment/evaluation of patients and families.
- Evaluate patient/family/caregiver emotional, social, financial, and environmental resources.
- Identify appropriate psychosocial problems, interventions and goals for care plans.
- Participate in development and implementation of initial and comprehensive care plans
- Regularly re-evaluate patient and family needs revising care plan as needed and communicating assessments to interdisciplinary team (IDT).
- Provide services to patients/families/caregivers in accordance with care plan. Provides services according to visit frequencies specified in care plan which are based on individualized, assessed needs of patient/family/caregiver.
- Prepare timely written evaluation/documentation supporting the clinical supervision of the Baccalaureate prepared social worker and submits the evaluation/documentation to the Team Director supervising the Baccalaureate prepared social worker.
- Master of Social Work (MSW) degree from a school of social work accredited by the Council of Social Work Education.
- Current and valid social work license (LMSW).
- Active social worker certification (CSW).
- Hospice or palliative care experience, preferred.
- Two (2) years of verifiable experience working as a Social Worker, preferred.
- Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
