The Admissions Nurse is responsible for facilitating patient transitions of care during admission which includes coordination with Central Intake, Clinical Manager, Program Director and Community Representative. A qualified admissions nurse is a licensed RN that will provide visit support to new admissions under our hospice care. This role requires working four 10-hour shifts, ideally Tuesday through Friday, from 9:00 a.m. to 7:00 p.m.

Key responsibilities of the Hospice Admissions Nurse:
- Provide nursing services to Care Initiatives Hospice patients, through visits that may include telephonic and in-person management of patients, families, caregivers and referral or community sources.
- Establish positive relationships with community sources with the intent to enhance and expedite patient admissions and coordination of care, medications, equipment, and supplies.
- Determine immediate needs and initial assessment, share findings with the RN-Case Manager and collaborate care with the IDT.
- Provide visits to assess ongoing patients’ needs, revise care plans as needed and communicate patient assessments to the team, when not doing admissions.
- Provide clinical services to patients and families that address goals of the care plan.
- Prepare clinical and progress notes; maintain clinical records; coordinate services and inform physician and other personnel of changes in patient condition and needs.
- Communicate with staff and necessary resources to coordinate services for patients and families.
- Current, valid Iowa registered nurse license or license in a compact state
- Two (2) years recent hospice or home care experience or admissions role in medical/surgical, oncology, or facility nursing.
- Current, valid CPR certification.
