Care Initiatives is seeking a Director of Centralized Intake. As the largest single operator of quality nursing homes in Iowa, Care Initiatives has over forty (40) skilled nursing communities, seven (7) hospice offices (and growing!), plus our own internal staffing agency. The Director of Centralized Intake will establish positive relationships with community sources with the intent to enhance and expedite resident referrals. The Director of Centralized Intake will also establish appropriate professional and public relationships that will encourage and develop short-term and long-term referral sources. In this role you will also lead an experienced area intake team while enhancing and coordinating the admission process for Care Initiatives.

Key Responsibilities Of The Director Of Centralized Intake
- Direct and oversee central intake team in driving post-acute referrals and admissions.
- Develop, implement, and monitor standardized protocols for clinical and non-clinical team activities. Facilitate integrated pro-active care review and management.
- Develop, perform, and promote interdepartmental integration, communication, and collaboration to enhance census growth.
- Collaborate with Vice President of Business Development, on centralized intake issues with staffing, systems, and program needs.
- Ensures monthly auditing on referrals, admissions, and denials.
- Develop, coach, and mentor centralized intake team.
- Bachelor’s degree in health-related field or equivalent work experience.
- Active RN license, preferred.
- Two (2) years of leadership experience.
- Strong understanding and experience working with Client Relationship Management Systems.
