The Hospice Clinical Documentation Coordinator is responsible for assuring accurate, timely and complete clinical records that support appropriate billing/reimbursement. This full-time role has the ability to be remote, hybrid, or in office.

The Hospice Clinical Documentation Coordinator is responsible for assuring accurate, timely and complete clinical records that support appropriate billing/reimbursement. This full-time role has the ability to be remote, hybrid, or in office.
What will a Hospice Clinical Documentation Coordinator do? Here are some key responsibilities:
- Serve as an internal reviewer for compliant hospice clinical documentation.
- Review patient charts for prognosis eligibility and technical requirements, according to direction from Leadership.
- Determine if clinical documentation, selected or assigned for review, supports reimbursement of hospice services and is compliant with billing regulations.
- Indicate if claims are billable/non-billable based on outcomes of technical and clinical documentation review.
- Conduct or support initial and ongoing training of clinical staff on hospice documentation processes and requirements, including the electronic and paper health record documentation.
- Work closely with compliance and finance to ensure clear collection and reporting of data.
- Conducts and reports results of regulatory mock surveys, including the participation in the follow-up reviews of plans of correction that result from regulatory reviews. Supports actual regulatory inspections/surveys.
- Responsible for clinical documentation review for internal or external audits, claim denials or appeals, including submission of Additional Development Requests (ADRs).
- Support other programs or other duties as assigned.
- Registered nursing education; bachelor’s degree preferred.
- Current, valid Iowa registered nurse license.
- Two years of recent clinical experience in hospice.
- Demonstrated understanding of electronic clinical record software, preferably MatrixCare.
