Job Description
We are seeking a qualified Case Manager/Social Worker to join our team. The ideal candidate will have a strong background in case management and experience working with individuals with disabilities.
Key Responsibilities:
- Provide case management activities necessary to meet the needs of clients assigned to the consolidated care team.
- Develop individualized care management plans and implement them through brokering and coordinating services.
- Monitor and evaluate clinical outcomes to ensure that services are provided in a professional, comprehensive, and cost-effective manner.
Requirements
To be successful in this role, you must have a minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
- Experience with Microsoft Office and state-maintained databases.
- Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.
- Maintain proficiency in company-sponsored training and certifications.
