Job Summary
The Support Coordinator / Service Coordinator / Case Manager provides case management activities necessary to meet the needs of clients assigned to the consolidated care team. This role involves developing individualized care management plans, implementing care plans through brokering and coordinating services, and monitoring and evaluating clinical outcomes.
- Develops Person-Centered Service Plans for each participant and modifies at least annually or when there is a significant medical or social change.
- Reports all incidents of participant abuse, neglect or exploitation immediately to supervisor/director.
- Actively coordinates with other individuals and/or entities essential in the physical and/or behavioral care delivery for the participant.
