Job Summary:
The DDD Support Coordination Supervisor is responsible for providing oversight, management and leadership to a team of Support Coordinators while ensuring agency compliance with New Jersey's Division of Developmental Disabilities (DDD) guidelines. They must adhere to designated internal supervision plan and act as the first point of contact for DDD staff and partner agencies. Supervisors for this program share responsibility for the DDD after hour's on-call phone line. This position combines leadership and oversight with quality assurance responsibilities to promote excellence in service delivery and adherence to all regulatory standards.
Key Responsibilities:
Supervisory Responsibilities:
Provide oversight of day-to-day operations of DDD Support Coordination Unit.
Lead, supervise, and mentor Support Coordinators to ensure high-quality, person-centered services.
Review and approve Individual Support Plans (ISPs) against the NJCAT, PCPT and ISP Guidelines for compliance, accuracy, and alignment with individuals' goals and needs.
Provide guidance on complex cases, supporting coordinators in problem-solving and service navigation.
Conduct regular team meetings to share updates, provide training, and address challenges.
Periodic Review of case records and documentation (both paper and electronic) pertinent to specific programs.
Preparation and facilitation of monthly team meeting agenda.
Assists administrative team with verification of monthly billing activity.
Ensure timely distribution of consumer satisfaction surveys and track rate of return and results.
Onboard, train, and support new Support Coordinators, ensuring they are equipped with the necessary tools and knowledge.
Maintain ongoing communication with individuals, families, guardians, and service providers to address concerns and ensure satisfaction.
Monitor service delivery quality and provide feedback to staff for continuous improvement.
Identify gaps in service delivery and collaborate with staff to address and resolve issues.
Analyze performance metrics and prepare reports for leadership to track progress and highlight areas for improvement.
Stay current with DDD policies and Medicaid updates, ensuring agency compliance and relaying changes to staff.
Train staff on documentation standards, person-centered planning, and best practices for compliance.
Complete all DDD required webinars and trainings
Qualifications:
Bachelor's degree in social work, Psychology, Human Services, or a related field (master's preferred).
Minimum of 2-3 years of experience in Support Coordination or related field, with at least 1 year of supervisory experience.
In-depth knowledge of DDD rules, Medicaid services, and person-centered planning in New Jersey is a plus.
Strong leadership, organizational, and analytical skills.
Experience in quality assurance, compliance, or performance monitoring preferred.
Proficiency in documentation systems, data analysis tools, and Microsoft Office Suite.
