The Care Management Coalition of Western New York, Inc. (CMC) consists of several nonprofit agency partners who share space at 1021 Broadway and collaborate in offering programs and services to the community. We are seeking a highly motivated and dedicated individual to join our team as a Case Manager that will report to the Managing Director. The successful candidate will play a vital role in connecting individuals and families in our community to the resources and support they need to thrive. Responsibilities
· Provide guidance and support to individuals and families seeking access to community resources, including healthcare, education, employment, housing, nutrition, transportation, etc.
· Manage inbound calls (inquiries) in a timely manner and route inquiries to the appropriate community resources
· Inquiries may include calls, emails, online applications, drop-in, etc.
· Compassionate and able to relate to different clients with various needs
· Motivation to encourage clients to follow-up on recommendations and care
· Strong verbal and written communication skills to explain to clients, family members and friends and professionals the case and care plan and maintain good case records
· Critical thinking and problem solving to determine the best care plan for each client after assessing clients’ needs
· Flexibility to change care plans if they are not getting the best results
· Organization to manage several different cases at once
· Computer literacy to document and maintain all case records in accordance with the program policies and procedures and supervisor guidance
· Ensure accurate data collection and entry of data.
· Participate in outreach and engagement activities to connect with the community and bring awareness to community resources
· Maintain accurate and up-to-date knowledge of community resources and referral processes
Qualifications
· Bachelor’s degree in human services, social work, or related field.
· 1-2 relevant experience (preferred)
· Experience working with underserved or marginalized communities (preferred)
· Excellent communication and interpersonal skills
· Ability to work independently and as part of a team
· Strong problem-solving and organizational skills
· Strong motivation to address the needs of diverse individuals and families and to improve organizations and systems
· The ideal candidate can complete all physical requirements with or without reasonable accommodations
Job Type: Full-time
Pay: $22.00 - $24.00 per hour
Expected hours: 40 per week
