Overview
The Hospice Care Coordinator plays a vital role in providing compassionate care to patients and their families during challenging times. This position involves coordinating hospice services, ensuring that patients receive the highest quality of care tailored to their individual needs. The ideal candidate will possess a strong understanding of hospice care principles, excellent communication skills, and a commitment to enhancing the quality of life for patients facing terminal illnesses.

Responsibilities

• Coordinate and oversee patient care plans in collaboration with healthcare professionals and family members.
• Conduct patient assessments, including vital signs monitoring and patient observation, to evaluate health status and care needs.
• Provide catheter care and assist with daily living activities, ensuring comfort and dignity for patients.
• Maintain accurate medical documentation and ensure compliance with hospice care regulations.
• Facilitate communication between patients, families, and interdisciplinary teams to address concerns and preferences.
• Support dementia care initiatives by implementing tailored strategies that enhance patient engagement and comfort.
• Assist in training staff on best practices related to hospice care systems and patient management.

Experience

• Previous experience in hospice care or nursing home settings is preferred.
• Familiarity with medical documentation processes and systems related to patient care.
• Skills in cooking and preparing meals for patients as needed.
• Experience in providing compassionate assistance to individuals with varying health conditions, particularly those requiring specialized attention such as dementia care.
• Strong observational skills to monitor changes in patient conditions effectively.

Pay: $30.00 - $40.00 per hour
