Job Summary
The Providence Center helps adults, adolescents and children affected by psychiatric illnesses, emotional problems and addictions by providing treatment and supportive services within a community setting
The Providence Center Child and Family Services Clinical Intake Specialist will provide outpatient psychotherapy either at The Providence Center and complete intake assessments for clients seeking services.
Duties And Responsibilities
Perform a comprehensive psychosocial assessment of assigned consumers.
Complete crisis assessments, initial assessments and integrated summary forms.
Develop and implement a plan of care with ongoing consumer input.
Provide individual mental health and/or substance abuse treatment and crisis assessment.
Perform record keeping in accordance with Health Information Services and other Center requirements.
Other duties as assigned.
Requirements
Master’s degree in Human Services, Social Work, or related field required.
Candidate must have OR be in the process of obtaining Independent license (LICSW and/or LMHC).
Experience Completing Clinical Assessments Preferred.
LCDP license and experience providing substance abuse treatment preferred.
Bilingual Spanish preferred.
Insured auto and valid driver's license.
