Job Summary:

The Providence Center Child and Family Services Community-Based Case Manager Enhanced Outpatient Services (CFS-EOS) supports clients engaged in enhanced outpatient services by addressing case management needs and coordinating care alongside outpatient therapists.

This role focuses on providing practical assistance, advocacy, and resources to clients and their families, ensuring holistic support for mental health treatment goals.

The position involves conducting community outreach, coordinating services, and fostering connections between clients and resources to promote stability and overall well-being.

Duties and Responsibilities:

Case Management Services

Conduct comprehensive assessments to identify client needs related to housing, education, employment, financial stability, healthcare, and community resources.

Develop and implement individualized service plans in collaboration with outpatient therapists and clients.

Connect clients to community resources, including social services, transportation, and financial assistance programs.

Provide ongoing monitoring and follow-up to ensure client needs are met and progress is achieved.

Collaboration with Clinical Staff

Work closely with outpatient therapists to align case management services with clients’ mental health treatment plans.

Attend team meetings and case reviews to share updates and collaboratively address client challenges.

Assist in crisis intervention planning and follow-up as needed.

Community-Based Support

Conduct home visits, school meetings, and community outreach to support clients in their natural environments.

Serve as a liaison between clients and community agencies, advocating for client needs and facilitating access to services.

Provide education and guidance to clients and families on navigating systems such as healthcare, education, and social services.

Documentation & Compliance

Maintain accurate, detailed, and timely records of all client interactions and services provided.

Ensure compliance with agency policies, ethical standards, and regulatory requirements.

Prepare reports and documentation as required for grants, audits, and program evaluation.

Requirements:

Associate's degree required, Bachelor's degree in Human Services or related fields preferred.

Minimum of 1-2 years of experience in case management, social services, or community-based work.

Experience working with individuals with mental health challenges and their families preferred.

Strong knowledge of community resources and systems, including housing, education, employment, and healthcare.

Excellent organizational, problem-solving, and time management skills.

Ability to build rapport and establish trust with diverse clients and community partners.

Cultural competence and commitment to providing equitable and inclusive services.

Proficiency in documentation and electronic record-keeping systems.

Valid driver’s license and auto insurance, reliable transportation, and ability to travel within the community.

Flexibility to work non-traditional hours, including evenings and weekends, as needed.
