Job Summary
The Licensed Practical Nurse (LPN) plays a vital role in supporting care management teams, leadership, and patients as a key member of the Transition Team. In this dynamic role, the LPN actively contributes to seamless patient transitions by promoting education, conducting personalized post-discharge follow-up calls, and ensuring a patient-centered experience. The LPN empowers patients with valuable post-discharge guidance, assists in coordinating follow-up appointments, and ensures timely and precise documentation within EPIC. Additionally, the LPN facilitates care coordination, working closely with the care team and practices to close any gaps in care, fostering an efficient and compassionate continuum of care for each patient.
Duties and Responsibilities:
Partner with Integra teams, including nurse care managers, social workers, PCPs, and skilled nursing facility staff, to ensure smooth care transitions for patients.
Identify and support patients needing care transitions by initiating the appropriate resources.
Provide compassionate telephonic and in-home support to enrolled members.
Conduct thorough assessments to understand and address healthcare needs.
Maintain accurate and timely care notes in Epic, adhering to department protocols.
Coordinate with physicians and community resources to deliver holistic patient care.
Serve as a valuable resource to community health workers and team members.
Participate in initiatives to enhance care management programs.
Recommend process improvements to optimize outcomes and efficiency.
Ensure professional and tailored communication for diverse audiences.
Adapt communication style to meet the needs of the situation and recipient.
Attend staff meetings and participate in required training and education sessions.
Strengthen internal and external partnerships through active collaboration.
Requirements:
Licensed Practical Nurse (LPN) with a minimum of 5 to 7 years of experience
Active RI LPN License required
Certified Case Management Certification required within 12 months of employment and/or eligible to test, Certified Diabetes Outpatient Educator preferred.
