Job Summary:
This dynamic role plays a key part in supporting the Nurse Care Manager by coordinating and collaborating on care management for complex patients. Acting as a vital link between the patient and the Nurse Care Manager, this position ensures seamless communication and care delivery. Additionally, it serves as the bridge between the Nurse Care Manager and external support services, driving connections that enhance patient outcomes and experiences.
Duties and Responsibilities:
Facilitates patient care coordination by connecting with outside support services, physician offices, and resources within Care New England, both telephonically and through in-home visits, ensuring patients receive the assistance they need.
Collaborates effectively alongside Nurses Care Managers (NCM), Social Workers (SW), and Resource Specialists to manage and support patients.
Supports NCM and ancillary team members in managing Complex Care Management patients, ensuring comprehensive care and coordination..
Performs audits as assigned by the Director of Care Management or their designee to ensure program standards and compliance are met.
Upholds patient confidentiality with the utmost care, preserving and protecting their rights in all interactions.
Assumes other related responsibilities as needed, demonstrating flexibility and commitment to team goals.
Requirements:
Vocational/Technical training and/or a minimum of five years job-related experience or equivalent is required.
Certified Medical Assistant.
