Job Summary
The Providence Center Residential Case Manager Atwood Street will provide clinical case management services in order to facilitate program goals and objectives using a quality care approach.
Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders.
Working with consumers to achieve the highest level of independence possible.
Schedule: Sunday second shift 3pm-11pm, Monday first shift 7am-3pm, Tuesday first shift 7am-3pm, Wednesday first shift 7am-3pm, Thursday OFF, Friday OFF, Saturday first shift 7am-3pm.
Duties And Responsibilities
Initiate treatment and advocate for consumer within network of care.
Negotiate and schedule appointments for consumers within The Providence Center and with outside agencies (Social Security Supplemental Insurance (S.S.I.), Food Stamps, Advocates Office, Medical Doctor, etc.).
Negotiate on behalf of the consumer, as needed, with landlords, courts, families, treatment team, consumers themselves.
Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities.
Develop professional relationship with Day Program staff.
Provide and maintain a home atmosphere.
Maintain and promote family contacts and relationships.
Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy.
Complete Required Interagency And Other Necessary Paperwork.
Complete shift activity logs.
Complete all required documentation with appropriate format.
Complete a clear, specific, behaviorally oriented treatment plan as required, with consumer involvement and signature.
Monitor confidentiality in accordance with Center policy and legal requirements.
Complete required reports and evaluations (ADL/DLA Assessments and Psycho-Social Functional Assessments).
Follow required office procedures in a cooperative manner.
Initiate and process incident reports as per Center guidelines.
Provide case management and support to consumers.
Treat consumers with respect.
Provide support counseling, problem solving, contracts and limit setting.
Establish attainable goals with the consumer.
Involve consumer, agencies and others in treatment planning.
Collaborate with clinical team on engagement insight and needed supports.
Provide support counseling with mental health issues, substance use disorders, and medical conditions.
Assist consumer in deciding which areas they want to address.
Assist and monitor the group home consumers with medical interventions and hygiene needs.
Encourage consumers to attain the highest possible levels of independence.
Accept the values and lifestyles of consumers.
Use a flexible approach with a diverse population.
Work flexible hours, including evenings, weekends, holidays and during inclement weather.
Make assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment.
Assess consumers' unmet needs (psychological, physical, housing, substance use, and psychiatric).
Identify, assess and respond to crisis situations.
Teach consumers adult daily living skills assisting them in all aspects of their life in the group home and supervised apartments.
Teach, supervise and assist consumers with doing household chores (morning and evening, laundry, room cleaning, etc.) using realistic situations.
Teach clients coping skills to implement symptoms management and harm reduction.
Provide direct alcohol and drug counseling/case management to individuals with substance use disorders.
Perform household duties as needed.
Use a step-by-step process when teaching skills.
Use positive reinforcement and encouragement.
Integrate skills training in all aspects of their treatment plan.
Maintain a clean, healthy and safe environment at all times.
Adhere to Infection Control protocols.
Implement emergency protocols when required.
Transport consumers in own vehicle or Center van to planned activities and appointments.
Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy.
Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
Other duties as assigned.
Requirements
High School Diploma/GED required, Associate's or Bachelor's degree preferred.
Experience In Residential Facilities Is Preferred.
Must have valid driver's license and insured auto.
Bilingual Spanish preferred.
