Key Responsibilities:
- Develop and implement individualized plans of care for patients
- Provide skilled nursing care to patients in need of home care services
- Accurately document care provided and maintain patient records according to agency policies and standards
- Participate in development of Plan of Care with Registered Nurse
Requirements:
- 1 year of Community Health Nursing experience
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Knowledge of Agency policies, State and Federal requirements, and JCAHO standards
