Responsibilities:

• Assess Patient Needs: Conduct thorough assessments to identify patient goals, objectives, and treatment plans, collaborating with the multidisciplinary team to ensure comprehensive care.
• Develop Treatment Plans: Create individualized treatment plans, incorporating evidence-based practices and addressing specific patient needs.
• Evaluate Patient Progress: Regularly evaluate patient progress, making adjustments to treatment plans as necessary, and documenting outcomes and concerns.
