Licensed Vocational Nurse
Overview
Join our team as a Licensed Vocational Nurse (LVN) where you will have the opportunity to provide exceptional care to patients in their home As an LVN, you will play a vital role in delivering high-quality nursing services and ensuring the well-being of our patients. This position offers a rewarding chance to make a positive impact on individuals' health and overall quality of life.
We currently have multiple patients in Lancaster and Palmdale area. Ideally, we are looking for an LVN to visit both.
Duties
- Administer medications and treatments as prescribed by healthcare providers
- Monitor and assess patient conditions, reporting any changes to the medical team
- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans
- Assist in patient education regarding health management and preventive care
- Maintain accurate and detailed patient records in compliance with regulatory standards
- Provide compassionate and empathetic care to patients of all ages, including children and elderly individuals
- Utilize eClinicalWorks or similar electronic health record systems efficiently
Qualifications
- Must hold a valid LVN license in CA
- Experience in hospital or primary care settings is preferred
- Familiarity with acute pain management techniques is advantageous
- Knowledge of health coaching principles is a plus
- Ability to work effectively in fast-paced environments while adhering to industry standards
- Experience in memory care or internal medicine is beneficial
- Willingness to travel for assignments if required
