**Job Responsibilities**
Develop and implement personalized care plans that meet the unique needs of our patients.
Collaborate with our interdisciplinary team to ensure seamless communication and coordination of care.
Assist patients with daily living activities such as bathing, dressing, and grooming.
Monitor patients' vital signs and report any changes to the RN or physician.
**Requirements**
Education: High School Diploma or GED required.
Experience: 1 year experience in patient care in a home health, long term care, hospice, and/or acute care setting preferred.
Current BLS certification required.
Possess valid state driver's license and reliable automobile and be willing to operate personal car necessitated by nature of job.
Proof of current automobile insurance with minimum state financial responsibility limits.
Current health certificate/physical examination and TB testing required.
