Registered Dietitian (Full-Time) | Skilled Nursing and Rehab Center

Care Pavilion Nursing & Rehabilitation Center is seeking a full-time dietitian to join their dynamic, compassionate and forward-thinking team!

Position Overview :

As the Registered Dietitian, you will oversee the nutritional well-being of our residents, conduct nutrition assessments and create effective nutritional care plans. We are excited for the right candidate to join our team and to support this role, we are offering a $10,00 sign-on bonus !

Registered Dietitian Key Duties :

• Develop individualized nutrition care plans based on patient assessments.
• Monitor and evaluate the effectiveness of nutrition services provided.
• Provide nutrition education, counseling, and support to long-term care residents and their families.
• Collaborate with interdisciplinary team members to ensure quality nutrition care for all residents.
• Ensure compliance with all state and federal regulations related to food service and nutrition in long-term care.
• Participate in quality improvement initiatives related to nutrition care in the facility.
• Maintain accurate and up-to-date documentation of nutrition care plans, assessments, and services provided.
• Evaluate the special needs of residents including nutrition support, dialysis, skin breakdown, and malnutrition.
• Partner with the interdisciplinary team to review and adjust the resident's plan of care.
• Work closely with speech-language pathologist to ensure appropriate mechanically altered diets are provided for residents with dysphagia.
• Perform other job-related duties as required.
