Job Summary:
As the Treatment for Individuals Experiencing Homelessness case manager, you will seek out unserved members of the community (individuals aged 18 and older, including aging-out youth and families with serious mental illness, serious emotional disturbances, and/or co-occurring disorder). You will work in the community locations where these members seek food, services, or temporary shelter, including the Bergen County Housing, Health, and Human Services Center, Bergen County HEARTS program, BCCWC, BCBSS, Bergen County Jail, and Bergen New Bridge Medical Center.
What You Will Do:
- Act as a "first responder," addressing complex crises involving both psychological and environmental components.
- Work holistically, ensuring that care and discharge plans meet the physical, social, and emotional needs of the client.
- Assist individuals with housing assistance
- Complete a comprehensive assessment of the clients' needs and identify resources, including basic needs and assist in accessing these services.
- Document all aspects of the services provided as well as creating a care plan with client.
- Link clients to community resources as needed.
What We Need to See:
- Bachelor's level degree in health, psychology, counseling, social work, education or other behavioral health profession.
- At least one-two years of experience working with the homeless population and /or with high need individuals with SMI/COD/SUD.
- Must possess the knowledge, skill, and experience necessary to competently perform case management activities.
- Live Experience preferred but not required.
- Bilingual Spanish preferred but not required.
- A valid driver's license and must be insurable by agency's automobile insurance.
