Care Plus TLC is searching for qualified Licensed Practical Nurses to join our team! Multiple 12 hour shifts are available and positions pay $34.00 per hour working up to 36 hours per week. Shift times are 8am-8pm. We are searching for dedicated nurses to care for 1 client in their home in Cedar Springs, MI.

- Responsibilities
- Take and record vital signs
- Monitor patient’s condition and report changes / concerns to appropriate medical professionals and family members
- Provide Activities of Daily Living care as outlined in the Care Plan-bathing, dressing, feeding, etc.
- Administer prescribed medications including injections, enemas / suppositories, etc as ordered by Physician
- Trach and vent care.
- Feeding tube care and Wound care
- No transfers!
- Repositioning and turning client in bed
- Provide emotional and psychological support when needed
- Understands First Aid and Emergency Care protocols
- Complete electronic documentation while on shift
- Skills
- Successful completion of LPN license program is a must
- 1 year experience as LPN
- Familiarity with home health, safety and sanitation standards and procedures
- Understanding of confidentiality and Privacy Laws and nursing best practices
- Ability to undertake a variety of diverse care tasks and work as a team
- Ability to learn electronic verification charting software
- Excellent communication and interpersonal skills
- Compassionate and able to handle tough situations if needed
- Trach and vent care experience is a MUST
