The Clinical Manager is an RN who provides day-to-day support to direct care team members (Nurses, CNAs, Therapists, etc.), including supervision and oversight of the patient’s plan of care, achievement of both clinical and administrative outcomes, and professional and supportive services to clients in a safe, cost-effective manner. The Clinical Manager is responsible for the coordination of care during the home health benefit period, staff engagement and communication, safety, and training of staff.
Duties Include:
- Collaborates with the clinical leadership team for overall quality of patient care and clinical performance of clinical staff.
- Reviews requests for services and determines patient eligibility/suitability for home care services.
- Oversees scheduling.
- Reviews documentation of other staff members and ensures missing, incomplete and/or untimely documentation issues are resolved.
- Assists clinicians in establishing immediate and long-term patient goals, setting priorities and developing Plan of Care (POC).
- Regularly evaluates the needs of the patient and makes necessary revisions to the plan of care in collaboration with clinicians.
- May provide direct patient care on a case by case basis.
- Manages the peer consultation process.
- Assists with training of staff to ensure quality care, compliance, and fiscal responsibility.
- Participates in the investigation and resolution of patient/family/physician concerns.
- Recommends employment decisions for staff to the Director of Nursing, including decisions to hire, fire, advance, promote or change status.
- Collaborates with the clinical leadership team in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
- Ensures compliance with federal, state, and local regulations
Qualifications Include:
- Licensed RN in VA
- Previous experience as a patient care or clinical manager
- Experience with CMS home health regulations and OASIS documentation
- Ability to provide constructive feedback and develop both clinical quality, core competencies, and productivity with direct reports
- Ability to seek patient feedback, understand the patient’s needs and advocate for them, and manage patient expectations
- Valid driver’s license and reliable transportation to perform job tasks
- Ability to work 9 AM – 5:30 PM Monday – Friday at our office in Fairfax, VA
- Wellsky (Kinnser) experience preferred
