Hospice Social Worker (MSW)
Salary: $75-$80k/yr (BOE)
Schedule: M-F| 8:00AM-5:00PM (on-call rotation)
Location: Toledo, Ohio and surrounding area
Requirements of the Hospice MSW role:
- Licensed Social Worker in state of practice
- Master's Degree in social work required
- One year of social work experience in a healthcare setting preferred
- Current Driver's License, vehicle insurance, and access to dependable transportation.
About the Hospice MSW role:
- Provide medical social services to the patient and/or family according to the Plan of Care.
- Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program, including emotional, social, financial, and environmental resources, and identify appropriate psychosocial problems, interventions, and goals for the patient's care.
- Participate in IDG meetings and activities and the development of the patient plan of care.
- Observe, record, and report changes in the patient's emotional and social factors that are being affected by the patient's illness and his/her need for care and response to treatment.
- Maintain and submit written clinical records as deemed by Care 360 Hospice, including the initial evaluation, the care plan, and daily notes.
- Confirm weekly, the scheduling of visits with the patient's case managers to facilitate coordination of other staff visits.
- Assess the patient's and family member's ability to cope with the patient's passing.
- Act as a consultant to the members of the health team, assisting them in understanding the social, emotional, and environmental factors related to the patient's health problems.
