This position reports to the Director of Care Management.
Role Summary and Responsibilities:
The Care Manager, LPN (LPN-CM) facilitates patient continuity of care with the healthcare team. Under the clinical direction and oversight of the Primary Care Physician and Registered Nurse, the LPN-CM coordinates care for patients needing support by collaborating with the patient, family, physician, nurses, and other members of the healthcare team to identify needs and expedite valuable, appropriate, and cost-effective care.
The LPN-CM will support patients recently discharged from the hospital for transitional care management and/or an individualized care plan focused on disease management and prevention, and the development of improved strategies to benefit patients at risk. They will engage patients and encourage them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices, adopt life-long healthy behaviors, close gaps in care, and reduce unnecessary ER utilization and hospital admissions/readmissions.
Key Responsibilities / Essential Functions:
The duties include, but are not limited to:
  • Implement and support individualized patient care plans.
  • Utilize tools and documents that support a guided care process, collaborate with patients/care givers toward an effective plan of care.
  • Address patient risk factors and/or obstacles to care including hospital/ER utilization.
  • Facilitate engagement between PCP and patient.
  • Demonstrated ability to influence others while motivating them to change.
  • Participation in required care management meetings or conferences.
  • Maintain predetermined patient caseload.
  • Exercise sound clinical judgment, seek advice and sign off from Provider or Registered Nurse as appropriate.
  • Take the initiative in finding solutions to difficult and/or sensitive problems.
  • Participate in quality gap closures.
  • Work as a patient advocate.
  • Provide mentoring/coaching of other pop-health team members as needed.
Non-Essential Functions:
  • Performs miscellaneous job-related duties as assigned.
Qualifications:
  • Must have a current NJ Practical Nurse License.
  • 2-5 years’ experience preferred in one or more of the following settings: Acute Care, Ambulatory, Specialty, Physician Office, or Home Health.
  • Navigation of local medical and social support preferred.
  • Previous experience with health systems and electronic health records
  • Must have valid driver license and ability to travel.
  • Willingness & ability to interact with healthcare personnel within a variety of clinical settings.
  • Understanding of HIPAA, Confidentiality Obligations and nursing best practice.
  • Demonstrated ability to work as part of a team.
  • Excellent interpersonal and organizational skills.
  • Candidate must be able to execute workflows and work together with RNs and other members of the multi-disciplinary care team.
  • Ability to review clinical data to address care opportunities and healthcare utilization trends.
  • Critical thinking skills & able to adapt to a fast complex working environment.
  • Strong autonomous problem-solving ability.
  • Excellent time management skills.
  • Multitasking and prioritization working in a high-volume environment with minimal supervision.
  • Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team.
  • Compassionate and able to handle stress.
  • Documentation accuracy in medical record.
  • Adaptable to new technologies and software.
Physical Requirements:
  • Mainly sedentary.
  • Sitting at the desk most of the day.
  • Standing or walking less than two hours per day.
  • Lifting no more than ten pounds on rare occasions.
  • Must be able to work at a computer and answer phone calls on a regular basis.

The Salary range for this position is
$31.50/hr. - $35.50/hr.
Compensation is based on the level and requirements of the role.
Salary within our ranges may also be determined by your education, experience, knowledge, skills, abilities, and location, as required by the role, as well as internal equity and alignment with market data.
