Job Overview
We are currently seeking Licensed Practical Nurses (LPNs) to join our team in New London County. As an LPN, you will have the opportunity to work independently in a home care setting, providing one-to-one patient care and making a tangible difference in the lives of our clients.
Responsibilities
- Perform home visits for patients in a home care setting
- Observe, record, and coordinate care with multi-disciplines regarding the patient's treatment and any change in condition
- Teach, supervise, and counsel the patient and family regarding home therapy procedures and other care needs as appropriate to the patient's condition
Qualifications
- Current license as a Licensed Practical Nurse in Connecticut
- Valid driver's license
- Reliable transportation and willingness to travel in New London County
- Strong communication and interpersonal skills
- Home health experience, including Medicare preferred
