- Provide high-quality nursing care to patients in their homes, focusing on prevention and early intervention.
- Collaborate with patients, families, and healthcare teams to develop and implement comprehensive care plans.
- Monitor patient progress, identifying areas for improvement and adjusting care plans accordingly.
Requirements:
- Must be a graduate of an accredited school of nursing.
- Must hold a current, active Registered Nurse license in the state of Massachusetts.
