Duties and Responsibilities:
- Personal Care: Provide individualized personal care to patients, including bathing, dressing, grooming, and toileting.
- Nutrition and Meal Planning: Plan and prepare nutritious meals tailored to meet the patient's dietary needs.
- Vital Signs and Monitoring: Take and record vital signs, such as temperature, pulse, and blood pressure, as ordered by the healthcare team.
- Ambulation and Exercise: Assist patients with ambulation and exercise according to their individualized plan of care.
- Medication Assistance: Support patients in self-administering medication, ensuring accurate dosages and adherence to treatment plans.
- Laundry and Housekeeping: Perform light housekeeping duties, including laundry, to maintain a safe and healthy environment for patients.
- Safety and Equipment Use: Ensure patients' safety needs are met, using equipment properly and safely (e.g., foot stools, side rails).
- Reporting and Documentation: Accurately document patient progress, reporting significant changes to the assigned nurse and adhering to organizational documentation standards.
Qualifications:
- Certification: Be a certified Home Health Aide (HHA) in the state of MA.
- Reliable Transportation: Own reliable transportation to ensure timely arrival at client visits.
- CPR/BLS Certification: Hold an up-to-date CPR/BLS certification.
- Linguistic Proficiency: Be fluent in English, both spoken and written.
- HIPAA Understanding: Comprehend HIPAA regulations and prioritize patient confidentiality.
- Cooking and Cleaning Skills: Possess basic cooking skills and be able to perform light housekeeping duties as needed.
- Education and Training: Complete a MA Health Department-approved HHA Training Program or demonstrate equivalent experience.
