Responsibilities:
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Provide direct patient care in accordance with state Nurse Practice Act.
- Communicate effectively with patients, families, and caregivers to ensure seamless care coordination.
- Maintain accurate and comprehensive records of patient interactions and care provided.
- Collaborate with healthcare professionals to ensure optimal care outcomes.
Requirements:
- Graduate of an accredited LPN or VN program.
- Current, active LPN licensure in Massachusetts.
- Up-to-date CPR/BLS certification.
- Minimum 3 years of nursing experience, preferably in community/home health or medical/surgical settings.
- Valid state driver's license and reliable vehicle.
- Authorized to work in the U.S. and provide proof of eligibility.
- Fluent in English, both spoken and written.
- Strong understanding of HIPAA regulations and commitment to patient confidentiality.
- Current health certificate/physical examination and TB testing results.
- At least 18 years of age.
