- Provide individualized care to patients in their homes, focusing on promotion of independence and improvement of overall health outcomes.
- Collaborate with multidisciplinary teams to coordinate care and ensure seamless transitions.
- Develop and implement effective care plans, tailored to meet the unique needs of each patient.
- Document patient progress and care provided, adhering to federal, state, and agency regulations.
Requirements:
- Degree from an accredited school of nursing.
- Current, active Registered Nurse license in Massachusetts.
- Up-to-date CPR/BLS certification.
- Valid state driver's license and reliable automobile.
- Authorizations to work in the U.S.
- Fluent English communication skills.
