Job Description
We are seeking a compassionate and dedicated Home Health Aide to join our team at CareConnect. As a Home Health Aide, you will provide high-quality care to patients in their homes, ensuring their safety and well-being.
Duties and Responsibilities:
- Provide personal care to patients, including bathing, dressing, and grooming.
- Assist with mobility and transfers to ensure patient safety.
- Administer medications as directed by healthcare professionals.
- Monitor vital signs and report any changes or concerns to medical personnel.
- Offer emotional support and companionship to patients and their families.
- Maintain a clean and safe environment for patients.
Qualifications:
- A Certified Home Health Aide (HHA) in the state of NY is required.
- Reliable transportation is necessary.
- A minimum of 35 hours/week and every other weekend availability is preferred.
- Eligibility to work in the United States is required.
- Fluency in English (speaking and understanding) is essential.
- Understanding of HIPAA regulations and patient confidentiality is crucial.
- Able to cook simple meals and perform light housekeeping duties as needed.
- Satisfactory completion of New York State Health Department-approved HHA Training Program or equivalency.
- Demonstrated adequate literacy and simple arithmetic skills.
- Age 18 or older.
