About the Role:

• Provide personal care services to patients, including assistance with daily living activities
• Collaborate with healthcare professionals to develop and implement care plans
• Ensure patient safety and well-being through regular monitoring and reporting
• Build relationships with patients and their families to provide emotional support and companionship
• Maintain accurate records of patient care and progress

Requirements:

• Certified Personal Care Assistant (PCA) or Certified Home Health Aide (HHA)
• Knowledge of HIPAA regulations and patient confidentiality
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills
• High school diploma or equivalent required
