We're seeking a skilled Licensed Practical Nurse (LPN) to join our team at CareConnect, providing high-quality care to patients in their homes. This role requires a compassionate individual who is dedicated to delivering exceptional patient care and support.
Responsibilities:
- Provide direct patient care in accordance with the state Nurse Practice Act.
- Implement the plan of care initiated by the registered nurse.
- Ensure accurate and timely documentation that aligns with the patient's plan of care.
- Assess and educate patients and their families/caregivers on self-care techniques and relevant information related to the patient's diagnosis and care plan.
- Coordinate home health services by communicating identified needs for other disciplines to the registered nurse and/or Clinical Supervisor.
