Job Summary
We are seeking highly skilled and compassionate Licensed Practical Nurses (LPNs) to join our team of dedicated healthcare professionals at CareConnect. As a key member of our care team, you will play a vital role in providing exceptional patient care in the comfort of their own homes.
About the Role
- Provide direct patient care services in accordance with state Nurse Practice Act standards.
- Implement and adhere to established patient care plans initiated by registered nurses.
- Maintain accurate and timely documentation of patient care activities.
- Educate patients and their families/caregivers on self-care techniques and relevant health information related to their diagnosis and care plan.
- Coordinate home healthcare services by communicating identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to registered nurses and/or Clinical Supervisors.
- Operate and maintain medical equipment and supplies effectively, ensuring adherence to aseptic techniques.
- Pursue ongoing personal and professional growth and development activities to enhance clinical skills and knowledge.
- Perform additional duties as assigned by registered nurses to support patient care and team collaboration.
Requirements
- Holds a current, active license as an LPN in the state of Massachusetts.
- Must have an up-to-date CPR/BLS certification.
- Has to have a minimum of three years of nursing experience, preferably in community/home healthcare or medical/surgical settings.
- Ability to provide an updated resume outlining relevant experience and qualifications.
- A valid state driver's license and reliable automobile.
- A strong understanding of HIPAA regulations and commitment to maintaining patient confidentiality.
- Current health certificate/physical examination and TB testing results.
- At least 18 years of age.
