What are the responsibilities and job description for the Licensed Practical Nurse - Loyal Home Care position at CareConnect?
We're looking for Licensed Practical Nurses in various locations in Worcester County, MA (See Below) to be at the heart of our client's care. As a Licensed Practical Nurse with Loyal Home Care Services, you'll need to be a licensed practical nurse in the state of MA, and provide exceptional care that our clients rely on to stay safe at home. Duties :
- Provide direct patient care in accordance with the state Nurse Practice Act.
- Implement the plan of care initiated by the registered nurse.
- Ensure accurate and timely documentation that aligns with the patient's plan of care.
- Assess and educate patients and their families / caregivers on self-care techniques and relevant information related to the patient's diagnosis and care plan.
- Coordinate home health services by communicating identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and / or Clinical Supervisor.
- Use and prepare medical equipment and supplies effectively, ensuring adherence to aseptic techniques.
- Participate in personal and professional growth and development activities to enhance clinical skills and knowledge.
- Perform additional duties as assigned by the registered nurse to support patient care and team collaboration.
