What are the responsibilities and job description for the Licensed Practical Nurse position at CareConnect?
We are seeking Licensed Practical Nurses in various locations across Worcester County, MA to play a vital role in the care of our clients. As a Licensed Practical Nurse with CareConnect, you will need to be a licensed practical nurse in the state of MA and provide exceptional care that our clients rely on to stay safe at home.
Duties:
- Provide direct patient care in accordance with the state Nurse Practice Act.
- Implement the plan of care initiated by the registered nurse.
- Educate patients and their families/caregivers on self-care techniques and relevant information related to the patient's diagnosis and care plan.
- Coordinate home health services by communicating identified needs for other disciplines to the registered nurse and/or Clinical Supervisor.
Qualifications:
- Must be a graduate of an accredited practical nurse or vocational nursing program.
- Must hold a current, active license as an LPN in the state of Massachusetts (MA).
- Must have an up-to-date CPR/BLS certification.
- Must have a minimum of three years of nursing experience.
- Must be fluent in English, both spoken and written, to communicate effectively with patients and the healthcare team.
- Must have a strong understanding of HIPAA regulations and demonstrate a commitment to maintaining patient confidentiality at all times.
