Key Responsibilities:
- Deliver high-quality patient care, adhering to the state Nurse Practice Act and established protocols.
- Collaborate with registered nurses to implement comprehensive care plans.
- Document patient progress accurately and efficiently, using industry-standard software.
- Educate patients and their families/caregivers on self-care techniques and relevant health information.
- Cultivate strong relationships with multidisciplinary teams to coordinate effective home health services.
Qualifications:
- A degree from an accredited practical nurse or vocational nursing program.
- An active LPN license in Massachusetts (MA).
- CPR/BLS certification.
- A minimum of three years' experience in nursing, preferably in community/home health or medical/surgical settings.
