Job Summary
We're seeking a skilled and compassionate Licensed Practical Nurse to join our team at CareConnect. Duties and Responsibilities:
- Provide direct patient care in accordance with the state Nurse Practice Act.
- Implement the plan of care initiated by the registered nurse.
- Ensure accurate and timely documentation that aligns with the patient's plan of care.
- Assess and educate patients and their families/caregivers on self-care techniques and relevant information related to the patient's diagnosis and care plan.
- Coordinate home health services by communicating identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Use and prepare medical equipment and supplies effectively, ensuring adherence to aseptic techniques.
- Participate in personal and professional growth and development activities to enhance clinical skills and knowledge.
- Perform additional duties as assigned by the registered nurse to support patient care and team collaboration.
Requirements:
- Must be a graduate of an accredited practical nurse or vocational nursing program.
- Must hold a current, active license as an LPN in the state of Massachusetts (MA).
- Must have an up-to-date CPR/BLS certification.
- Has to have a minimum of three years of nursing experience.
- Experience in community/home health or medical/surgical settings is preferred.
- Must provide an updated resume outlining relevant experience and qualifications.
- Must have a valid state driver's license and reliable automobile.
- Must be authorized to work in the U.S. and provide proof of eligibility if necessary.
- Must be fluent in English, both spoken and written, to communicate effectively with patients and the healthcare team.
- Must have a strong understanding of HIPAA regulations and demonstrate a commitment to maintaining patient confidentiality at all times.
- Must provide current health certificate/physical examination and TB testing results.
- Must be at least 18 years of age.
