CareConnect is seeking a Patient Advocate to join our team of dedicated caregivers. As a trusted member of our team, you will work closely with patients and their families to deliver personalized care that enhances their quality of life.
Duties and Responsibilities
- Provide emotional support and companionship to patients and their families
- Assist with daily activities, such as meal preparation and light housekeeping
- Support patients with mobility and transfer needs
- Monitor vital signs and report any changes or concerns to medical personnel
- Maintain a clean and safe environment for patients
Qualifications and Requirements
- Must be a Certified Home Health Aide (HHA) in the state of SC
- Able to work a minimum of 12 hours/week and every other weekend
- Eligible to work in the United States
- Fluent in English (able to speak and understand)
- Understanding of HIPAA regulations and the importance of patient confidentiality
