What are the responsibilities and job description for the Patient Advocate position at CareConnect?
Patient Advocacy is at the heart of what we do at CareConnect. As a Patient Advocate, you will work closely with our patients and their families to ensure they receive the care and support they need.
Key Responsibilities:
- Participate in personal and professional growth and development activities to enhance clinical skills and knowledge.
- Perform additional duties as assigned by the registered nurse to support patient care and team collaboration.
Qualifications:
- Must be a graduate of an accredited practical nurse or vocational nursing program.
- Must hold a current, active license as an LPN in the state of Massachusetts (MA).
- Must have an up-to-date CPR/BLS certification.
- Has to have a minimum of three years of nursing experience. Experience in community/home health or medical/surgical settings is preferred.
