Main Responsibilities:
- Providing personalized care services tailored to each patient's unique needs
- Assisting with daily living activities, including bathing, dressing, and grooming
- Supporting patients with mobility and transfer needs
- Administering medications as directed by healthcare professionals
- Monitoring vital signs and reporting changes or concerns to medical personnel
Requirements:
- Certified Personal Care Assistant (PCA) or Certified Home Health Aide (HHA)
- Strong knowledge of HIPAA regulations and patient confidentiality
- Ability to prepare simple meals and perform light housekeeping duties as needed
- Satisfactory completion of New York State Health Department-approved HHA Training Program or equivalency
- Demonstrated proficiency in basic arithmetic and literacy skills
- Age requirement: 18 years or older
