Job Description
We are seeking a compassionate and dedicated Personal Care Professional to join our team as a Patient Support Specialist.
Key Responsibilities:
- Provide exceptional patient care and support in personal hygiene, grooming, and daily living activities.
- Measure and record vital signs, report any deviations, and assist with medication management under nurse supervision.
- Aid patients with mobility tasks, such as transferring to and from beds, chairs, and assisting with walking aids.
- Perform light housekeeping tasks, including laundry, dish washing, and maintaining the patient's living environment.
- Effectively communicate with patients, families, and healthcare team members while maintaining accurate records of care and patient status.
Requirements:
- Certification as a Home Health Aide (HHA) with completion of a training program approved by the relevant board of nursing.
- Good standing, holding a current, valid certificate as a Home Health Aide issued by the relevant board of nursing.
- Completion of the Home Care test.
- Age 18 or older.
- Reliable transportation to and from patient locations.
