About the Role:
In this critical role, you will work closely with our clients to understand their unique needs and preferences, and develop personalized care plans that meet their physical, emotional, and social needs.
Key Responsibilities:
- Providing personal care services, including bathing, dressing, and grooming
- Assisting with daily living activities, such as meal preparation and medication management
- Supporting clients with mobility and transfers, as needed
- Monitoring and reporting any changes in client condition to assigned nurse or supervisor
- Maintaining accurate records and documentation of client care
Requirements:
To be successful in this role, you must possess:
- A valid Certified Home Health Aide (HHA) certification in the state of MA
- An up-to-date CPR/BLS certification
- Excellent communication and interpersonal skills
- The ability to work independently and as part of a team
- A strong commitment to providing exceptional patient care
