- Maintain accurate and timely documentation of patient progress and communicate changes to care teams.

Requirements
- Current active license as an LPN in Massachusetts (MA).
- Up-to-date CPR/BLS certification and valid driver's license required.
- Minimum three years of nursing experience in community/home health or medical/surgical settings.
- Strong commitment to upholding HIPAA regulations and maintaining patient confidentiality.
- Ability to adapt to changing situations and priorities in a fast-paced home health care environment.
