As a Rehabilitation Specialist, you will be responsible for evaluating patients, developing and implementing therapy plans, and collaborating with healthcare teams to achieve optimal patient outcomes.
**Required Skills and Qualifications:**
* NY Professional License and current registration as a Physical Therapist
* Minimum 1 year of full-time experience providing physical therapy services
* Ability to assess and respond to patient and family needs in a home care setting
* Home care experience preferred but not required
