Your Role as a Registered Nurse:
You will be the clinical cornerstone for our patients, providing expert leadership and care within their homes. As an RN, you will assess, plan, and manage patient care while collaborating with a multidisciplinary team committed to quality.
Key Responsibilities:
*Conduct comprehensive initial and ongoing assessments based on individual patient needs and provider orders
*Develop and continually update care plans to reflect evolving conditions and goals
*Deliver skilled nursing interventions within the scope of practice and in accordance with physician directives
*Maintain detailed and compliant documentation per all state, federal, and agency regulations
*Lead case conferences, promoting collaboration between caregivers and clinicians
*Provide culturally sensitive education to patients and families regarding condition management and treatment plans
What We Need:
*Graduate of an accredited Nursing Program (RN)
*Active Registered Nurse license in MA
*Valid CPR/BLS certification
*Reliable transportation & valid driver's license
*1 year of experience in acute care, home health, outpatient, or community health (within the last 2 years)
*Up-to-date health certificate & TB test
*Clear understanding of HIPAA and strong commitment to confidentiality
*Fluent in English (spoken & written)
