About the Role
- Perform initial and ongoing patient assessments based on patient needs and physician orders.
- Develop and update written plans of care, ensuring they are re-evaluated as necessary.
- Lead case conferences to discuss patient progress and future care plans with multidisciplinary teams.
What We Offer
