Duties
- Evaluate patient's physical, cognitive, and functional abilities
- Create and implement individualized treatment plans
- Deliver evidence-based therapeutic interventions
- Educate patients and families on safe, independent living strategies
- Collaborate with interdisciplinary healthcare teams
- Maintain accurate documentation and patient progress records
- Active NYS Occupational Therapy License
- Relevant experience (home health preferred)
- Strong communication and organizational skills
- Ability to work independently in patient's homes
- Starting at $105,000 salary;
- $75 per visit!
