- Job Summary: We are looking for a dedicated Physical Therapist to provide high-quality physical therapy services to patients in their homes.
- Responsibilities:
Assessment and Direct Care: Evaluate patients for functional limitations and develop personalized therapy plans to address their needs. Provide therapy services to treat medical and surgical conditions using accepted physical therapy modalities.
Education and Communication: Instruct and supervise patients, families, home health aides, and others on home exercise programs and aspects of patient care. Report significant clinical information to physicians, clinical managers, and other team members. Document clinical evaluations, treatments, and patient responses to therapy.
Qualifications: NY Professional License and current registration as a Physical Therapist. Minimum 1 year of full-time experience providing physical therapy services. Ability to assess and respond to patient and family needs in a home care setting. Home care experience is preferred but not required. Ability to use technology (such as tablets) for patient documentation and care. Experience with Oasis assessments is a plus. A valid driver's license and access to a reliable automobile for travel in the field.
