About the Role:
As a Social Service Designee, you will play a vital role in ensuring the emotional, social, and psychological well-being of our residents. You will work closely with residents, families, and interdisciplinary teams to provide support, coordinate services, and enhance quality of life in our facility.
Responsibilities:
- Assist in assessing residents' psychosocial needs and develop individualized care plans
- Provide emotional support and counseling to residents and families
- Coordinate services such as discharge planning, community resources, and advocacy
- Maintain accurate documentation in compliance with state and federal regulations
- Collaborate with nursing and therapy teams to enhance resident care
- Assist with admissions, resident rights education, and grievance resolution
Qualifications:
- Experience in social services, preferably in a healthcare or long-term care setting
- Knowledge of state and federal regulations regarding resident rights and social services
- Strong communication, organization, and problem-solving skills
- Ability to build relationships with residents, families, and staff
- Bachelor’s degree in Social Work or related field (preferred)
